See the EQ-5D: Link to the measure and information on how to purchase

The EQ-5D is a widely validated standardised outcome measure. It can be used with a wide variety of conditions, treatments, and professions. It provides a simple descriptive profile and single index value for health care status, with an optional add-on to assess economic evaluation. The EQ-5D measures patient health across the five domains: mobility; self-care; usual activities; pain/discomfort; and anxiety/depression. The EQ-5D takes only a few minutes for the patient to complete. It can be completed during or after the consultation and can be completed as part of a patient-postal survey, meaning that the impact on clinic times is minimal.

It is widely used in clinical and economic evaluations of healthcare and in population health surveys. It is validated across several countries, so various language translations are available. It is not free to use, however – see the link below for more details.

The EQ-5D website has comprehensive, clear and easy to follow information about how to obtain and use the EQ-5D and how to interpret and report the findings. We recommend that you should visit their website if you are interested in using this measure. The EQ-5D website is easy to navigate and very easy to use.
